By agreeing to this form, you agree to the following:

1. Client is responsible for any damage to Muse Photo Booth Co. equipment caused by guests, event attendees, or venue conditions.

2. This includes (but is not limited to):

   • Broken iPad

   • Damaged booth housing

   • Spilled drinks on electronics

   • Pulled cords

   • Knocked-over lighting

   • Destroyed props/backdrops

3. Normal wear-and-tear is NOT considered damage.

4. If damage occurs, the client may be billed for repair or replacement costs not to exceed the value of the damaged equipment.

5. By checking this box, the client confirms they are the responsible party for the rental period.
